    What are the UMID ID Requirements?

    To process your UMID application, you will need a duly filled UMID application form and a copy of one valid government-issued ID (e.g. Passport, Driver’s License, Voter’s ID, NBI Clearance, Seaman’s Book). In case you don't have any of the primary IDs mentioned, an original and photocopy of two secondary IDs; containing your photo and your signature must be provided:

    • Birth certificate
    • Marriage contract
    • Taxpayer's Identification Number (TIN) ID
    • School or company ID
    • Senior citizen card
    • Police clearance

    How do I apply for a UMID?

    Step 1: Prepare the required documents and forms

    Make sure you have photocopies of your primary and secondary IDs with you. Next, fill out the UMID Card Application Form and print a copy of it.

    Step 2: Schedule an appointment or walk in at any SSS branch

    Visit the SSS website to set an appointment at your desired date and branch. You can visit select SSS branches that allows walk-in applicants.

    Step 3: Complete all your requirements and register your biometrics

    Please submit your needs to the designated personnels. At the enrolling station, they will check your documents and take your photo and biometric information which includes your signature and fingerprints. Once the biometrics have been completed, you will receive an acknowledgment receipt.

    Step 4: Get your card

    Congratulations on completing your UMID application! All you have to do is wait for a message from SSS that your UMID card is ready for claiming. You can also check the status of your UMID application in online portal under UMID/SSS ID Details. 

    With your UMID card in hand, you now have one of the most powerful IDs in the Philippines, simplifying transactions with SSS, GSIS, PhilHealth, and Pag-IBIG.
